Fill in the form below and we’ll send you a easy referral pack, including 50 patient referral cards, a poster for your practice and an email pack to start referring today. If a patient needs psychological support, just write a referral to I relate. Health and hand them a patient referral card. Your patients can scan the QR code and book in for their first session.
